Patient questionnaires

The questionnaires sent to one patient, what they have returned, and whether their anamnesi is still valid.

This page gathers everything to do with one patient’s questionnaires: which are still valid, which have been sent, and which have come back.

What you can do here

  • Send a questionnaire to the patient, if you have the permission.
  • Check validity status — whether this patient’s anamnesi is current or needs collecting again.
  • See what has been sent, including when, and when the link expires.
  • Resend a link that has not been used.
  • Read the answers to a submitted questionnaire.
  • View the audit trail for a sent link.

The three sections

Validity status is the summary you want first: it tells you whether this patient’s anamnesi still stands, rather than making you infer it from a list of sends.

Sent lists the links issued to this patient, each with the date it went out and the date it expires. A link is how the patient reaches the questionnaire; they fill it in without logging in to Kairo.

Submitted lists what the patient has actually returned, with the date. A submission records the version of the questionnaire it was captured under, so an answer stays readable against the questions that were actually asked, even after the template has moved on.
